DATE OF CRASH: July 14, 2015

LOCATION: 2400 block of Veterans Pkwy West
CR#: 15-011364

VEHICLE 1 (V-1): A 2014 orange Dodge Mini-Van (Journey).
DRIVER 1 (D-1): Nora Maria Lopez, W/F, 67, of Hialeah, FL.
DRIVER 1 INJURIES: Lopez did not sustain any injuries from the traffic crash.
PASSENGER 1 (P-1): Grisel Maria Muina, W/F, 68, of Miami, FL.
PASSENGER 1 INJURIES: Muina did not sustain any injuries from the traffic crash.

VEHICLE 2 (V-2): A 2001 Harley Davidson Motorcycle (Road King).
DRIVER 2 (D-2): 56 year old W/M.
DRIVER 2 INJURIES: Driver 2 died at the scene as a result of the injuries he sustained from the traffic crash.

SYNOPSIS: V-1 was traveling westbound in the eastbound lanes of Veterans Pkwy at the 2400 block. As the D-1 was driving, she realized she was on the wrong side of the road and stopped her vehicle. D-1 then began to make a three point turn and was diagonal (facing southwest) across the merger lane and outside lane of Veterans Pkwy. At the same time, V-2 had just made a left turn onto Veterans Pkwy from Surfside Blvd and was now traveling eastbound in the outside lane of Veterans Pkwy.

As V-2 was traveling, D-2 suddenly saw V-1 traveling in reverse, across the outside eastbound lane. D-2 then took evasive actions, attempting to swerve into the inside eastbound lane to avoid V-1. As D-2 was taking evasive actions to avoid V-1, the front end of V-2 struck the right rear of V-1. D-2 was then totally ejected from V-2, striking the ground in the inside eastbound lane and sliding to final rest near the median. After the collision, V-2 fell to its left side and slid to its final rest in the inside eastbound lane. The collision caused V-1 to rotate clockwise and come to final rest in the eastbound merge lane of Veterans Pkwy.

D-1 and P-1 did not sustain any injuries as a result of the traffic crash. D-2 sustained fatal injuries and was pronounced dead at the scene.

Due to the injuries D-2 sustained from the crash, THI Cpl. N. Green, Forensics, VA, and the Medical Examiner Investigator responded to the scene of the crash.

Alcohol/Drugs are NOT a factor in the traffic crash for D-1.
